CHRISTUS Santa Rosa Family Health Center Awarded Recognition by the National Committee for Quality Assurance Physician Practice Connections – Patient Centered Medical Home

The CHRISTUS Santa Rosa Family Health Center has once again been awarded Level 3 Recognition by the National Committee for Quality Assurance (NCQA) Physician Practice Connections ® - Patient Centered Medical Home™ (PPC-PCMH). The Family Health Center has two locations: inside the Goldsbury Center for Children & Families on the Children’s Hospital of San Antonio campus and inside Medical Plaza 3 on the CHRISTUS Santa Rosa Hospital – Westover Hills campus.  The Center was the first and only practice in Bexar County to receive such a distinction and is one of only four Family Medicine residency programs in the state of Texas to achieve the highest level of recognition possible.

The patient-centered medical home is a model that CHRISTUS Santa Rosa Health System is committed to. This care model, provided by physician practices, seeks to strengthen the physician-patient relationship by replacing episodic care based on illnesses and patient complaints with coordinated care and a long-term healing relationship. The physician-led care team is responsible for providing all the patient's health care needs and, when needed, arranges for appropriate care with other qualified physicians. A medical home also emphasizes enhanced care through proactive management of patients with chronic diseases, open scheduling, and communication between patients, physicians, and staff.

To receive Level 3 recognition, which is valid for three years and is the highest recognition possible, the CHRISTUS Santa Rosa Family Health Center demonstrated the ability to meet the program's key criteria embodying characteristics of the medical home.  The standards are aligned with the joint principles of the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association.

The CHRISTUS Santa Rosa Family Health Center provides comprehensive care for the entire family, from infants to senior citizens, and provides an array of services including routine check-ups, care for acute and chronic illnesses, minor surgical procedures, family-centered maternity care, sports medicine, gynecological/well-woman care, and access to on-site laboratory and imaging services.

The Family Health Center has utilized an Electronic Health Record (EHR) since 2005 and also provides its patients with access to the clinic’s web-based Patient Portal, where patients may contact their physicians and their health care team via online secure messaging; access and download their medical history information, laboratory and imaging results; and explore a vast library of health information and wellness tools.
